1. Identify the parties
Enter the exporter or shipper and the consignee using the same names and addresses shown on the related shipment paperwork.
2. Add shipment references
Include invoice, shipment, transport or customer references that help connect the certificate to the correct transaction.
3. Describe the goods
List clear product descriptions, quantities and tariff or HS references when they are required for the transaction.
4. State the country of origin
Show the country of origin for the goods or line items using the origin determination required for the shipment.
5. Complete certification
Add the required statement, authorized signer, title and date, and obtain chamber or other certification when the specific process requires it.
6. Reconcile related documents
Check the certificate against the commercial invoice and packing list so parties, products and shipment references agree.
